To prepare for completing the National Sport Academy application package, please have the following information before you begin to complete the application. ONCE YOU BEGIN COMPLETING THE APPLICATION YOU WILL NOT BE ABLE TO SAVE THE INFORMATION AND RETURN TO IT LATER.

Report Card. Please have the student athletes most recent report card. Email it to: [email protected]
Individualized Program Plan (IPP). If the student athlete has an IPP, please provide a copy of the IPP to: [email protected]
Sport Resume. Please provide information such as: your sport goals / dreams, where you have played / competed / trained for the last 3 - 5 years, who your coaches have been during this time / their contact information, any individual recognition or team awards you may have received and any other information you would like us to know. Current Coach - Name & Contact Information.
Character reference name - the name of the FIRST person you have contacted and chosen to provide a reference for you.
Character reference name - the name of the SECOND person you have contacted and chosen to provide a reference for you.
Parents / Guardians - names and contact information
Emergency Contact - name of the emergency contact person(s) and contact information.
Medical Information - doctor's name and contact information, allergies or important medical information you want the National Sport Academy to be aware of.

Photo / Video Release Policy *

I have read and understood the National Sport Academy Photo / Video Policy.

The National Sport Academy uses photographs and video to produce educational and marketing / promotional tools. National Sport Academy photos and video which may include National Sport Academy student athletes may be used for this purpose and will be displayed on the National Sport Academy website, Facebook page and on Instagram. Photos and video may be kept on file at the National Sport Academy office for future use.

Transportation/Equipment Policy For Athletes Training With The National Sport Academy (if applicable) *

When student athletes begin their day at a National Sport Academy training session, parents are responsible for their student athlete’s transportation to the training facility.
When student athletes begin their day at school, parents are responsible for their student athlete’s transportation to school.
When student athletes train within the school timetable – not first thing in the morning and when the National Sport Academy training facility is not within walking distance, the National Sport Academy will provide transportation from the school to the training facility; after the training session the National Sport Academy will provide transportation from the training facility back to the school for their next class.
When student athletes end their day at a National Sport Academy training session. Parents are responsible for their student athlete’s transportation home from the training facility.
When student athletes end their day at school. Parents are responsible for their student athlete’s transportation from school to home.
All National Sport Academy student athletes are required to supply all of their own personal training equipment. Please discuss with your National Sport Academy sport coaches if there are any specific equipment requirements.

Financial - Fees & Payment Timelines

National Sport Academy Deposit - Payable At The Time The Application Is Submitted *

A deposit of $750.00 is required at the time the application is submitted. Send an e-transfer to [email protected]. (Note, there is no "s" after sport in the email address). Applications will not be considered processed until the deposit is received.

Prior to August 15, 2025 you will be provided a reminder of the schedule for future payments. The schedule for future payments on the balance of the National Sport Academy fees is:

August 15, 2025 - 25 % of the balance
September 15, 2025 - 25 % of the balance
October 15, 2025 - 25 % of the balance
November 15, 2025 - 25 % of the balance
(Payment can be made in full)

Prior to August 15, September 15, October 15 and November 15, the National Sport Academy will email you a reminder of the payment due.

Terms & Conditions Of Enrollment *

Once a National Sport Academy student athlete has been accepted into the National Sport Academy, no portion of the fees, paid or outstanding, will be refunded or cancelled in the event of the student’s injury, absence, withdrawal or dismissal from the National Sport Academy.

Send the link ( https://nationalsportacademy.ca/character-reference/ ) to the two people you have chosen to provide a character reference for you. Follow up with your character reference to ensure they have completed the reference form.
Attend a National Sport Academy training session as our guest to experience a training session. The National Sport Academy will work with you to coordinate this.
The National Sport Academy will schedule a meeting / interview with you to review the National Sport Academy program and answer any questions you may have.

Once the eight steps above have been completed, you are considered to have a complete National Sport School application. The next step is the National Sport Academy will be in touch with you to discuss your enrollment in the National Sport Academy.
