• In preparation for the National Sport Academy Lacrosse travel, please note the following information and complete the form.

• To ensure the National Sport Academy is able to choose the best flights and receive the best group reservation fees, we will begin to explore flights immediately.

• The information entered in this form will be provided to the airline for airline tickets. Please enter the information carefully. Names and birthdates need to match EXACTLY to what appears in the person’s passport. Inaccurate information will result in DELAYS at the airline check-in counter.
